Data & Knowledge Management:
The Framework for Health

“The key to integrated care is integrated data.”

Data is at the heart of effective commissioning. Managing a population’s health requires the connected health data of individuals across all the care services.

The NHS is sitting on a vast wealth of patient data but at present it is fragmented and disconnected. And as much of it is regressive, there is little chance for PCTs to glean knowledge that will help them forward plan their services. GPs have lots of data on primary care but no link to data in secondary and acute care. Public Health Authorities have good data on acute care but lack data on primary care. Humana’s data and knowledge management systems extract data available from primary, secondary, pharmacy, dental and mental health sources. Once the data is harvested, it is amassed in Humana’s data warehouse where the ‘white noise’ is removed. A unique set of tools and algorithms are applied in order to reveal pertinent information that can be turned into usable knowledge. This allows for a complete evaluation of the interplay of various intervention strategies and their likely impact in terms of cost and process enhancement.

A combination of predictive modeling, data visualisation and other techniques are then implemented. These identify immediate and longer-term risk factors for ill health; assess concordance with evidence-based best treatments; and evaluate whether current treatment is consistent with available evidence. The same rigor is further applied to management systems, ensuring compliance, quality and efficiency.

Humana’s integrated data and knowledge management solutions provide decision makers with timely, accurate and pertinent information. Thus the ‘finger will always be on the commissioning framework pulse’. With a firm foundation based on clarity and increased certainty, the commissioner is then free to make accurate forecasts to plan and design future strategies.

Benefits of Humana’s data and knowledge management services:

For Patients

  • Early identification of predisposition to preventable conditions
  • Opportunities for positive behaviour change
  • Community support in line with population needs

For PCTs

  • Comprehensive, factual analysis to facilitate commissioning decisions
  • Accurate prediction of population’s future healthcare needs for resource development planning
  • Quality control of service provision
